How Do I Issue a Red & Yellow Card?


Red and Yellow Cards are part of a disciplinary procedure used to document unsafe actions and behaviours in the workplace. This procedure supports the regulation of employee conduct and assists in decisions regarding removal or termination from site, particularly in cases where site rules have been breached. It also serves to raise awareness among workers about the consequences of their actions and encourages greater compliance.


  1. A Red Card signifies a serious breach of health and safety procedures and may lead to removal from the workplace, depending on the severity of the incident. For example, a Red Card may be issued if an employee operates machinery without having received the necessary training.
  2. A Yellow Card represents a formal warning for actions or behaviours that, while less severe, still have the potential to cause harm. This may result in a warning being issued and additional measures being put in place to prevent recurrence. For instance, a Yellow Card may be issued if an employee fails to wear the appropriate PPE for a task.


  1. On the left column, click Red & Yellow Cards.
  2. Click +New Red & Yellow Card.
  3. Enter a Brief Description.
  4. Choose who the employee the card will be issued to and who it was issued by.
  5. Enter the Occurrence Date.
  6. Choose the Evaluation Form from the dropdown options
  7. Tick the box if you wish to allow Administrators and Employees to sign the report.
  8. Choose the Project and Location relevant to the Red/Yellow Card.
  9. Click Save & Submit.


Once this has been set up, the Evaluation Form can be filled and the required signatures can be added to complete the set up. The system will keep a record of Red & Yellow cards that are added to the system, unless they are deleted.


This is required before you can add a Red & Yellow Card:

  1. Digitise your Evaluation Form
  2. Set up Projects
  3. Set up Locations
  4. Set up Notifications


  1. A field with a red asterisk beside it means it is required and you will not be able to complete your work without filling in these fields i.e. description, issued to and evaluation form.


  1. A field without a red asterisk beside it means the field is not required, although it may be helpful to fill these out to complete your work i.e. issued by, date, project and location.


Other Options:

Under the dropdown options, you might see an option to add more, by clicking the Here button highlighted. This will take you to the section you wish to add more of, i.e. if you want to add more evaluation forms, projects and locations to the system - be cautious that when you click this button, the details you have entered will be deleted.
