How Do I Issue a Red & Yellow Card?


Red & Yellow Cards are disciplinary procedures. They are issued to employees to document unsafe actions and behaviours.

A red card represents a serious breach of health and safety procedures and can result in removal from the workplace, depending on the severity of the action. An example of a red card being issued is if an employee is operating machinery without adequate training.

A yellow card represents a warning for actions or behaviours being carried out that are less serious but could still cause harm. This can result in the employee receiving a warning and procedures being put in place to prevent the action from happening again. An example of a yellow card being issued is if an employee is not wearing the correct PPE for a job.


  • On the left side menu, click ‘Red & Yellow Cards’.
  • Click ‘+ New Red & Yellow Card’.
  • Enter a Brief Description.
  • Choose the employee the card will be issued to and who it was issued by.
  • Enter the occurrence date.
  • Choose the Evaluation Form from the dropdown options
  • Tick the box if you wish to allow Administrators and Employees to sign the report.
  • Choose the Project and Location relevant to the Red/Yellow Card.
  • Click ‘Save & Submit’.


  • A field with a red asterisk beside it means it is required and you will not be able to complete your work without filling in these fields i.e. description, issued to and evaluation form.


  • A field without a red asterisk beside it means the field is not required, although it may be helpful to fill these out to complete your work i.e. issued by, date, project and location.


This is required before you can add a Red & Yellow Card:

  • Digitise your Evaluation Form
  • Set up Projects
  • Set up Locations


Other Options:

Under the dropdown options, you might see an option to add more, by clicking the ‘Here’ button highlighted. This will take you to the section you wish to add more of, i.e. if you want to add more evaluation forms, projects and locations to the system - be cautious that when you click this button, the details you have entered will be deleted.
