How Do I Add a Safety Observation?


Safety Observations are recorded to identify both safe and unsafe behaviours, as well as potential hazards in the workplace. Reporting safety observations helps to prevent incidents at work and supports compliance with health and safety regulations.


This article includes step-by-step instructions on how to add a Safety Observation.


  1. From the home screen, tap Services at the bottom of the screen.
  2. Tap Safety Observation.
  3. Tap Add New at the top of the screen.
  4. Tap on the Type you would like to add.
  5. Enter a Brief Description about the safety observation.
  6. Enter the Occurrence Date.
  7. Tick the box(es) if:
  8. The initial SOR was raised previously.
  9. The SOR has already been completed.
  10. Select the Priority.
  11. Select who the Safety Observation was reported by from the dropdown options.
  12. Choose the Location and Project from the dropdown options.
  13. Choose the Subcontractor from the dropdown options - the Subcontractor must be assigned to one or more locations to be able to use the system, as subcontractors need this information to set up employees or accept jobs.
  14. Pinpoint the location of the safety observation on the map by clicking on the location.
  15. Tap Next.
  16. Fill out the Report Form.
  17. Click Save Changes.
