How Do I Edit a Digitised Form?


If you want to edit a saved Digitised Form, click 'Open Form' at the top of the page and select the form from the dropdown options.


  • On the top right corner, you will see a lock icon, if you click this you will be able to move the tools around and place them in the order you want.
  • Beside the lock icon there is a toggle icon, if you hover over this you will see the options ‘Duplicate’, ‘Edit’ and ‘Delete’.
  • If you click ‘Duplicate’, this will make a copy of the tool.
  • If you click ‘Edit’, this allows you to make changes.
  • If you click ‘Delete’, this will remove the tool, and it cannot be recovered.
  • When you are editing a tool, you will see the options under the ‘General’ heading, these allow you to make changes to the text such as: make the text bold, italic, underline etc.
  • When you have finished editing, click ‘Close’.


Common editing options include:

  • You can choose if you want a tool to be visible or hidden by clicking the boxes. If you choose hidden, the tool will only show based on the field and values you select.
  • Tick ‘Required’ if the text input is required, the person filling out the form cannot complete it without this.
  • Tick ‘Centre Text’ if you want the item to be in the centre.
  • Tick ‘Disabled’ to make the item read only.
  • Tick ‘Centre Text’ if you want an item to be in the centre.
  • Tick ‘Allow block next’ to block the next tool from being filled.
  • Tick ‘Allow N/A’ to make an item not applicable.
  • Tick ‘Bold’ or ‘Italic’ if you want the heading of an item to be bold and/or italic.


On the right column, you will see the heading ‘Toolbox’, with a selection of tools you can use to create your form:

 

The Section tool allows you to create multiple sections for your form.

  • To add tools to the section, drag the tool you wish to add from the right column, and drop it into the section.
  • Note – you cannot drag and drop a tool into the section if it is already in the form, and you cannot take a tool out of the section once it has been added.

 

  • The Header Text tool allows you to create a header for your form.

 

  • The Paragraph tool allows you to input information.


  • The Link tool allows you to add a clickable link to the form.


  • The Line Break Tool allows you to create a break in the form.

 

  • The Text Input tool allows the user to enter text, this can be relevant to the title you enter for the text input.


  • The Date tool allows the user to enter a specific date that is relevant to the form.


  • The Number Input tool allows the user to enter a specific number using the arrows on the right side of the field.

 

The Multi-Line Input tool allows the user to enter information.

  • You can choose the number of rows for the input by entering the number into the ‘Rows’ field.

 

The Range tool allows the user to select an amount using the bar option.

  • You can select the minimum and maximum range using the arrows on the right of these fields when editing.

 

The System Field tool shows specific information based on what you select.

  • Select what you want to show from the dropdown option and the count reports.

 

The Maths tool allows the user to make calculations based on what you have selected.

  • Select the field to operate with from the dropdown options.
  • Choose the operator from the options.
  • The arrows on the left allow you to change the fields location.
  • You can add a field by clicking ‘+Add Field’ or remove a field using the bin icon on the left.


  • The Body tool allows the user to select a specific part of the body that has been affected.

 

The Checkbox tool allows the user to make multiple selections based on what you have entered.

  • Enter the question or statement into the text box.
  • You can import specific data from the system by selecting from the dropdown options. Based on what you select will automatically show when the user is filling out the form.
  • You can add multiple options under the ‘Options’ heading.
  • To add more options, click the plus icon on the right.
  • To remove options, click the minus icon.

 

The Radiobutton tool allows the user to make a single selection based on what you have entered.

  • Enter the question or statement into the text box.
  • You can import specific data from the system by selecting from the dropdown options. Based on what you select will automatically show when the user is filling out the form.
  • You can add multiple options under the ‘Options’ heading.
  • To add more options, click the plus icon on the right.
  • To remove options, click the minus icon.

 

The Dropdown tool allows the user to make a selection using a dropdown.

  • Enter the question or statement into the text box.
  • You can import specific data from the system by selecting from the dropdown options. Based on what you select will automatically show when the user is filling out the form.
  • You can add multiple options under the ‘Options’ heading.
  • To add more options, click the plus icon on the right.
  • To remove options, click the minus icon.

 

The Tags tool allows the user to make multiple selections.

  • Enter the question or statement into the text box.
  • You can import specific data from the system by selecting from the dropdown options. Based on what you select will automatically show when the user is filling out the form.
  • You can add multiple options under the ‘Options’ heading.
  • To add more options, click the plus icon on the right.
  • To remove options, click the minus icon.

 

The Signature tool allows the user to enter a digital signature.


The Multiple Signatures tool allows the user to enter multiple signatures.


The File tool allows the user to upload a file.


The Files with Status tool allows the user to upload a file which will show the status of the file i.e. approved, rejected, expired, about to expire.


The Image tool allows the user to upload an image.


The Attach Documents tool allows the user to upload a document.

 

The Table tool allows the user to fill out the contents based on what you have entered.

  • Enter the name into the ‘Table’ text box.
  • To add more fields, click the plus icon, to remove a field click Remove.

 

The Attach Voice Note tool allows you to upload a voice note you have already recorded.


The Voice Notes tool allows the user to upload a voice note.
