How Do I Configure a Safety Observation Type?


Safety Observations are recorded to identify both safe and unsafe behaviours, as well as potential hazards in the workplace. When setting up a Safety Observation, you assign it a Type, which includes the Initial and Investigation Report Forms. These forms must be digitised before configuring Safety Observation Types.


To Configure a Safety Observation Type:

  1. On the left side menu, go to Settings and click Safety Observation Types.
  2. Click +New Safety Observation Type.
  3. Enter the Name and Description of the Safety Observation Type.
  4. Choose the Initial Report and Investigation Report Forms from the dropdown options.
  5. The Initial Report Form is filled when setting up a Safety Observation and the Initial Report Form is filled after the Safety Observation has been set up.
  6. Enter the Quotas that apply into the textboxes.
  7. These are put in place to ensure that a required number of safe or unsafe acts are recorded within a given timeframe.
  8. Click Save & Submit.


This is required before you can add a Safety Observation Type:

  1. Digitise your Initial and Investigation Report Form


  1. A field with a red asterisk beside it means it is required and you will not be able to complete your work without filling in these fields i.e. name, description and initial report form.


  1. A field without a red asterisk beside it means the field is not required, although it may be helpful to fill these out to complete your work i.e. investigation report form and minimum quota.


Other Options:

  1. Under the dropdown options, you might see an option to add more forms, by clicking the ‘Here’ button highlighted. This will take you to the form builder section - be cautious that when you click this button, the details you have entered will be deleted.
