The Safety Observations Module


Safety Observations are recorded to identify both safe and unsafe behaviours, as well as potential hazards in the workplace. Reporting safety observations helps to prevent incidents at work and supports compliance with health and safety regulations.


Recording Safety Observations – Initial Safety Observation Report (Stage 1)

When first adding a safety observation, the system requires basic details, such as the type, date, location, and the relevant project. Once this information has been entered, users can complete the investigation report forms that are linked to the selected safety observation type.

Users may also submit safety observation forms by scanning a QR code using their phone’s camera. When scanned, a link will appear that directs them to the public safety observation forms. This is particularly useful as not all users may have immediate access to a computer.

QR codes can be generated using the ‘Public Forms’ feature within the Safety Observations module.


Recording Safety Observations – Safety Observation Investigation (Stage 2)

Completing an investigation form is an optional step when logging a safety observation. These forms allow for more detailed information to be added regarding the observation. An investigation report form can be assigned when configuring a safety observation type in the system, and will then be available when that type is selected.

Safety Observation investigation report forms can be digitised using the ‘Form Builder’ functionality.


Safety Observation Statuses

Administrators are able to approve or reject safety observations to ensure they are thoroughly and accurately documented.

The statuses available are:

  1. Approved
  2. Awaiting Approval
  3. Rejected


Safety Observation Quotas

Safety observation quotas are put in place to ensure that a required number of safe or unsafe acts are recorded within a given timeframe. These quotas can be set daily, weekly, monthly, or annually. This feature is available within the Settings menu.


Corrective actions can be implemented to safety observations to identify the root causes of issues.
