How Do I Add a Fault Report?


The Fault Reports Module is designed to document faults that occur with machinery, ensuring that detailed records are kept for both initial issues and investigations. This module helps track faults and provides the necessary documentation for analysis, investigation, and maintenance follow-up.


When a fault occurs, the initial reporting stage must be completed first. An administrator can then complete the fault report form, which captures detailed information needed for further investigation and analysis.


Fault Report Forms Include:

  1. Initial Fault Report Forms: Used during the first stage of reporting.
  2. Investigation Fault Report Forms: Used to capture detailed insights during the follow-up investigation.


Fault report forms can be digitised using the Form Builder functionality, making it easier to complete and store fault reports.


This article includes step-by-step instructions on how to add a Fault Report.


  1. On the left side menu, click Fault Reports.
  2. Click +New Fault Report.
  3. Select the Fault Report Type from the dropdown options.
  4. Enter the Tag Number - this is the number used to identify the Fault report
  5. Select the Fault Report Classification from the dropdown options.
  6. Enter the Occurrence Date.
  7. Select the Machineries from the dropdown options (you can select multiple).
  8. Click Next.
  9. Fill out the Initial Report Form - this is the form that was added to the Fault Report Type.
  10. Click Save Changes.
  11. Fill out the Investigation Report Form by clicking on the Fault Report.
  12. When you have completed the Investigation report form, click Save Changes.


This is required before you can add a Fault Report:

  1. Create the Fault Report Types
  2. Create the Fault Report Classifications
  3. Set up Machineries
  4. Set up People
  5. Set up Notifications


  1. A field with a red asterisk beside it means it is required and you will not be able to complete your work without filling in these fields i.e. Fault report type, tag number, classification, occurrence date, engineer, machine location and fault description.


  1. A field without a red asterisk beside it means the field is not required, although it may be helpful to fill these out to complete your work i.e. Machineries.


Other Options:

Under the dropdown options, you might see an option to add more, by clicking the ‘Here’ button highlighted. This will take you to the section you wish to add more of, i.e. if you want to add more types, classifications and machineries to the system - be cautious that when you click this button, the details you have entered will be deleted.
