How Do I Document an Issue and Opportunity?


An Issue and Opportunity can be added to the system to help identify any issues in the workplace and improvements that are needed. On the system, you will be able to see the status as well as other details about issues and opportunities that have been added. You can implement forms when adding issues and opportunities to include any other relevant information.


  • On the left side menu, go to 'Settings' and click 'Issue & Opportunities'.
  • Click ‘+ New Issue & Opportunity’.
  • Enter the title.
  • Choose the Issue and Opportunity Type from the dropdown options.
  • Choose the Company from the dropdown options.
  • Choose the Location and Departments from the dropdown options (you can select multiple).
  • You have the option to save the issue and opportunity anonymously by ticking the box.
  • Click 'Save & Submit'.


When you have completed adding the issue and opportunity, you will need to fill out the Issues and Opportunities Form.

  • You can access the Issue and Opportunity you have created by going to the main page and clicking on the title.
  • Scroll down to the Forms section and click the down arrow beside the heading.
  • Fill out the form by entering the information and click save.
  • When you have completed filling in the form, click 'Save Changes'.


The Corrective Actions section is just below the form, you can add corrective actions to the issue and opportunity by clicking add to immediate or add to long term.


  • A field with a red asterisk beside it means it is required and you will not be able to complete your work without filling in these fields.


  • A field without a red asterisk beside it means the field is not required, although it may be helpful to fill these out to complete your work.


  • This is required before you add an Issue and Opportunity:
  • Create an Issue and Opportunity Type
  • Set up Companies
  • Set up Locations
  • Implement Corrective Actions


Other Options:

  • Under the dropdown options, you might see an option to add more, by clicking the ‘Here’ button highlighted. This will take you to the section you wish to add more of, i.e. if you want to add more types, companies and locations to the system - be cautious that when you click this button, the details you have entered will be deleted.
