The Red and Yellow Cards Module


Red and yellow cards are issued to subcontractor employees as part of disciplinary action. This procedure helps to regulate employee behaviour, and can be helpful when making decisions on employee removal/termination when employees have broken site rules.

This procedure also helps bring awareness to employees of the consequences of their actions and can encourage compliance among employees.


Red and Yellow Cards are issued depending on the action or behaviour that took place, such as:



Safety violations

  • A yellow card is given as a warning for minor safety violations, focusing on the need for improvement without severe consequences.
  • A red card is given as a serious penalty for major safety violations, which may lead to removal from site.


Behavioural and performance

  • A yellow card is given as a formal warning for inappropriate behaviour, poor performance or non-compliance.
  • A red card is issued for repeat offences or serious misconduct, which may result in termination.



Documenting Red and Yellow Cards

When adding a red and yellow card, the system requires information, such as the employee, date, form, project, and location. When this information is added, an administrator fills out the form that was added to the red and yellow card. Forms can be configured using the ‘Form Builder’ module.


It is possible to select whether an administrator signature and employee signature should be taken into account for form completion (“Required”) or not. Employees can sign red and yellow cards that are assigned to them by logging in to their portal.



Red and Yellow Card Status and Score

The Statuses of red and yellow cards are shown on the main page of the Red and Yellow Cards module. 


The statuses that show are:

  • Completed – the required forms have been filled.
  • Not completed – forms need to be filled.
  • Missing admin signatures – an administrator needs to sign the form.
  • Missing employee signatures – an employee needs to sign the form.


Red and Yellow Card scores are configured using the Form Builder functionality. When forms are completed, the total score can be accessed from the Red and Yellow Cards main page.


Corrective Actions

Corrective actions are implemented to identify the root cause of risks and can help prevent risks from recurring. 


Type of Corrective Actions 

There are two types of corrective actions that can be implemented:

Immediate - steps taken to quickly resolve the issue

Long Term - steps taken to improve overall processes to prevent recurrence 


Adding corrective actions includes:

  • Documenting the issue - include the problem description and any relevant details
  • Identifying the root cause - what caused the issue
  • Implementing action(s) that is required to solve the issue


When implementing action(s) to a corrective action, it can be helpful to implement SMART actions:

  • Specific - enter exactly what needs to be done to resolve the issue
  • Measurable - include what is needed to track progress of actions being taken
  • Achievable - set a realistic but effective goal 
  • Relevant - make sure the actions align with the objectives
  • Time - Bound - set a time frame to ensure the actions are completed quickly
