To access Public Forms:

  1. On the left menu, click the relevant module.
  2. Click Public Forms at the top of the page.
  3. Using a mobile device, scan the QR code using the camera.
  4. Click on the link that appears.
  5. Fill out the form, ensuring all the required details are added.
  6. Click Save Changes.
